Registration of Self-Employed Persons

Section 13 of the National Insurance and Social Security Act, 1966, makes provision for the insurance of self-employed persons who are over 16 years of age and under pensionable age.

All self-employed persons not previously registered must now complete the Application for a National Insurance Number Registration Form. This revised form includes information from the previously used Application by Self-Employed Person for National Insurance Card (DP13).

Please ensure that numbers 1-14 and 18-20 are completed. The completed form must be signed and returned to the National Insurance Office, Frank Walcott building, Flodden, Culloden Road, St. Michael together with an original birth certificate Barbados identification card, and in the case of a married woman, a marriage certificate.
